Web Form: Question/Answer Types

There are various different choices for the Answer Type of a question.

                                             


 

The set of Question Types includes:

  • Check - A single checkbox.  This type of question cannot be "not required".
  • Checks - A set of checkboxes where any collection of the boxes can be checked, from none to all.
  • Comment Box - A text area.
  • Date - A date.  User selects a date from a Calendar popup.
  • Department (PCR)  -  Department.  User selects a department from a drop down list of departments.
  • Generic Code
  • Location -  Location.  User selects onelocation from a drop down list of locations.
  • List - A custom list of choices that is defined as part of the question definition itself.
  • No Answer - A "question" that is really just explanatory or other text.  No answer is collected.
  • Radio Buttons - A set of radio button options where the user selects one of the options.
  • Text Box - A text area.  A single row of text with a defined number of displayed characters and a maximum size.
  • Picture - An image that is uploaded from a file and shown as part of the Form.  No answer is collected.
  • Employee Form Signature - Employee signature capture.  A request for the staff member to sign the form.
