Recommended Employee Forms Settings -
We recommend adding an approval chain for the individuals who verifies the information and sign the form (the certifying officer and the supervisor of the certifying officer). This will not only ensure the form is reviewed, it will electronically add the approvers signature to the PDF form along with the approvers name and date the form was approved. The Certifying Office must be the first approver of the form and the Certifying Officers Supervisor must be the second approver.
Also, to change the telephone number and/or extension, please edit the PDF Form as noted below:
