The Evaluations Conference Forms screen lists all of the conference forms defined in the system. "Conference Forms" is now a bit of a misnomer. A "Conference Form" is really any time of data collection (input) form.
Form 'usage' – Controls which drop down – mostly on the Modify Workflow screen- the form appears in. Each form has only one usage.
