Web Form: Checks Question Type

To create a question where the user can select from a set of checkboxes (rather than just one), use the "Checks" Answer Type:


                               


This answer type requires you to edit the question.  When you click "Add" to add the new question, it is added to the list of questions as shown below - but it is incomplete:



You must add the set of checkboxes you need.   Click the Edit button to open the question for editing.  At the bottom of the screen, there is a control to add the list of options:



Enter your set of options one at a time by entering the Seq number for the option, which specifies its place in the list, and then the text of the Item:

                          



There is no limit to the number of items that can be displayed.

To preview the "Checks" answer type, view the "Preview" screen for the Web Form:

                          


The "Checks" Answer Type displays with the question to the left and the set of check boxes to the right, each with its own label.  The user can select any combination of the checkboxes, including all or none of them.
