What do the employees see in their portal when completing the form?
Evaluation Conference Form - Can be imported from the Library
Output to PDF Form - Can be imported from the Library
Required PDF Form modification -
Navigate to the PDF Forms list, search for the form and click on the Fields button
Scroll to the last row, replace 'CHANGE ME IN PDF FORM" with the PERS Location number for your District and click Save.
