To add a question which displays a single checkbox, select "Check" as the Answer Type:
When you click "Add", the new question appears in the list of questions:
To edit the question and update its properties and function, click on the "Edit" button. This will bring up the full "Modify Question" screen:
To preview a question, click on the "Preview" tab that is displayed when a question is being edited:
The "Check" Answer type displays as a single checkbox with the question to the left of it and no label to the right:
