The 1095C is the Department of the Treasury's "Employer-Provided Health Insurance Offer and Coverage" form. Details regarding this form can found here.

Genesis SchoolFi Payroll provides support for the creation and printing of these forms.
- There is a 1095C screen in staff member's personnel Payroll screens (Personnel→Modify Staff→Payroll→1095C).
- Data for filling the form is taken from the staff member's demographics information and their health benefit information as recorded on their Health Care screen (Personnel→Modify Staff→Health Care).
- Mass 1095C functions are provided on the Payroll 1095C article (Payroll→1095C).
- Most staff members will have 1095C records generated for them. Only if a staff member has no medical deductions and was not offered coverage will they not get a 1095C (or if the staff member did not actually hold a position in the district).

The SchoolFi Payroll 1095C function calculates and fills in the information for Part II of the form, lines 14, 15, and 16:

The information in Part III of the form, Covered Individuals, the list of the staff member's covered individuals, is taken from the staff member's Payroll > Health Care screen.
Information for Part I of the 1095C form is taken from staff member's demographics.
