Utilizing the Signing Statement for a timesheet

This text will appear the pin number field in the Sign and Submit popup dialog box in the employee portal when an employee is submitting their timesheet.

Sample Signing Statements utilized by Districts:

I hereby certify that this time sheet contains an accurate and true statement of my actual hours worked on-site during this pay period. I am aware that any submission of intentionally inaccurate time sheets will constitute a crime, for which I may be subject to civil and criminal penalties, and the immediate termination of my employment with the XXXXXXXXX School District.

I, THE UNDERSIGNED, DO HEREBY CERTIFY THAT THE ABOVE IS A TRUE RECORD OF SERVICE RENDERED BY THE EMPLOYEE WHOSE NAME APPEARS ABOVE AND WHOSE SIGNATURE IS AFFIXED, AND THAT THE SERVICE RENDERED WAS IN ACCORDANCE WITH PRIOR AUTHORIZATION RECEIVED FROM THE SUPERINTENDENT OF SCHOOLS AND/OR THE SECRETARY / BUSINESS MANAGER.

I attest that services claimed on this form are accurate, that the time claimed was used in the actual instruction of the child, and that the adult above was in attendance during the time that service was rendered.
