Employee Forms can only be deleted if they have not been used in the School Year. Forms are school year specific, so they can be rolled over from year to year, but the Employee Form definition can only be deleted in a year in which it has never been used.
If an Employee Form has not been used, it will display a button at the far right side of its entry in the list. If the button exists, clicking it will bring up a confirmation dialogue:
Click Ok to remove the Employee Form
