EEO-5 Survey Report

About This Report:

The Equal Employment Opportunity Survey is a survey that is performed every 2 years from a select number of districts with 100 or more employees.

This report will produce an Excel file or a CSV file that can be used to fill in Part 2 of the EEO-5 168A Form Survey.

Report Data:

Staff that are included in this report are:

  • All employees that have an employment status of Active
  • All employees with an active Employment History Record as of the report parameter date
  • All employees that have the EEO-5 Classification field or the 2nd EEO-5 Classification field set (See Below)

The EEO-5 Classification codes are broken into 3 groups on the report:

  • Full Time Staff
  • Part Time Staff
  • New Hires Full-Time

Each classification code contains the counts of male and female ethnicities/races for all staff included above with that classification selected. Staff with both classification fields set will count towards both classification codes.

Columns are calculated as follows:

Hispanic M/FTotal male or female staff with the "Hispanic" box checked in Personnel → Staff → Demographics, regardless of any other race checked.
White M/FTotal male or female staff with only the "White" box checked in Personnel → Staff → Demographics.
Black M/FTotal male or female staff with only the "Black or African American" box checked in Personnel → Staff → Demographics.
Asian M/FTotal male or female staff with only the "Asian" box checked in Personnel → Staff → Demographics.
Pacific M/FTotal male or female staff with only the "Hawaiian native/other Pacific Islander" box checked in Personnel → Staff → Demographics.
Native M/FTotal male or female staff with only the "American Indian/Alaskanbox checked in Personnel → Staff → Demographics.
Two+ M/FTotal male or female staff with any combination of 2 races above checked in Personnel → Staff → Demographics, not including the Hispanic ethnicity.

This federal survey does not track nonbinary as a gender option, so staff must be designated male or female to be counted in the report. 

Report Audit:

The report will generate an Audit with a list of all active staff, their EEO-5 classification(s), and their ethnicity/race as calculated above. Staff without an EEO-5 classification set will have the phrase "* No Code *" in the Assignment column.

Data Entry Required:

This report requires that all staff with an employment status of Active have the EEO-5 Classification field or the 2nd EEO-5 Classification field set.

These fields are found on the Personnel → Staff → Modify → Employment History Screen.
